I believe this is of the "petite head" variety.

Would this go PO1, or PO details?

The details grade might be cool too. It would be like the lowest of the low.

I have no idea what that is sticking out of the eyebrow area.

Normally I'd agree, but that black dent right in the center of the bust looks pretty bad, plus we don't have the reverse. Poor/Fair details, damaged IMHO.
