1985 Nickel Dollar Error, Off-Center, Missing Reeded Edge

I'm certainly not an error expert, so I will post my images and let the experts explain this one.

There is no reeding on the edge, and I'm guessing the damage on the rim is from the ejection process? Also seems slightly underweight but I'll have to double check with another scale.

Looks like an uncentered broadstrike, faulty collar die deployment...lets see what the experts say

Thanks everyone. I was sorting piles of junk and world coins when I found this in a bag of modern American coins. I asked my ol' man about it, he shrugged and said "keep it!"

It was a small fee after spending most of my holidays organizing his 'junk' piles into nice 1 lb bags ;) I use the word 'junk' loosely.
