This looks to me to be another centered broadstrike, 20mm wide. It is interesting how different the obverse is to the reverse. Maybe someone can explain what it is we are looking at and how it happened. I am interested to know what goes wrong with the minting process.

Looks like it was in a bezel because the extra width is slated. I need to see what the experts say.

It looks like a broadstrike on a blank ("Type I planchet"). The reverse shows some "rim" development (not the actual design rim) due to transient contact with the collar. I see no indication of post-strike damage/alteration.
