It may have been placed between two pieces of leather and tightened down on part of the coin in a vise. That would alter just one area. It couldn't of happened at the mint as the image is distorted. If it were a mint error, the design would be normal shaped.
Genuine double-struck "sandwich strikes" do exist. These are coins struck a second time between two planchets. However, the coin you referenced does not resemble the genuine article. As Coop said, it was crushed outside the Mint.
Correct me if I'm wrong, but I would think the presence of an appropriate rim (before it got smeared) would pretty much rule out anything strike-related, right?
