It is not old roman,but mediėval Germany, Hungaria, Bohemia (I think)
I have several similar coins.
You have to be a real expert to give a correct determination of such coins (and I am not)
Dat can be between 1300 and 1500

Here you can see many examples of this strange embossing technique peculiar to medieval Germany, producing a raised central square superimposed on the coin's image:
