It appears to be a thin metal impression (or electrotype) of the reverse side of a coin from Retegno, a commune within the Duchy of Milan, of Antonio Teodoro Trevulzio, minted between 1676 and 1678. The shield features a crowned head with three merged faces, between the words VNICA and MENS:
