I couldn't see a zoomed in picture from the front. However, the way the red on the back right edge seems to keep the bill's pattern, IMHO I think it is ink.

Could be either or. but personally I would be more concerned about that hole that looks like a burnt mark on the seal.

Certainly not blood; that does not stay red forever.
Perhaps the color has persisted, because it is pigment (inorganic) or dye (organic) based.

Lipstick, thinned paint or ink could be liable candidates.
If it is dye, it could be bleached out, but that would cause further permanent problems.

Don't clean banknotes!

In a serious vein, I think sel's analysis is correct.

My question is, why does the cause matter on a note in this condition?
