Hard to take pictures in the 2 by 2. Looks to be in fairly high grade. I will remove if necessary for better ID. The opening in the 2 by 2 is for a US Quarter.

Hard to see with the glare but I am certain that this is Maximianus.

Reverse type CONCORDIA MILITVM minted at Cyzicus

Hard to tell in the flip, but may be a follis of Maximianus, with a "Jupiter presenting Victory on a globe" reverse (CONCORDIA MILITVM).

The coin looks fine:

I agree that it is of Maximian, but it is not a "follis" it is a "post-reform radiate" which at the time was tariffed at about a half follis if memory of RIC VI serves me.
