It's not an error.

Someone just ground the copper layer off exposing the zinc core.

In my humble opinion, this in my experience would be a Dryer Coin, it got stuck in the dryer and worn both sides pretty even.....

After thinking about this, I am wrong I think, i'be pulled pennies out of the washing machine pump that resemble your coin.....
