1964-D Roosevelt Dime Damage

I went through a possible OBW roll of 64-D dimes today and found this interesting specimen with severe damage. No other dimes in the roll were damaged.
I'd like to learn more about how this happens. I assume it hung up somehow in the process, and slipped through quality control.

Well first things first: It's not an error, like you mentioned.


Most likely flattened by something, probably ran over by a car.

If I have posted this in the wrong place, I apologize, and mods please move or delete.
I should have mentioned in the op that all 50 coins in the roll were uncirculated.
