Could be a misprint label.

It's a "12" series holder (ca. 2000)

Cert number looks OK.

I'm not seeing a problem. Have you verified the number with PCGS?

Verifying the numbers doesn't help much when authenticating a slab, unless there is a TrueView image. Counterfeiters include legitimate cert numbers on their fake slabs.

I;d say the slab is real, just a misprinted label. The printer just didn't feed properly between the first and second line.
