Found this coin today with what looks like a smaller S East of the larger S. Or is this a well placed PMD.

I think it is coincidental circulation wear, but very cool to look at.

Note the difference in size between the two areas. A mint mark punch only makes the same size of punch mark. It is PSD from something that made contact with the coin. The raised area with the light at a certain angle makes it create a shadow. (12:00) and the image from a different angle just show the crater.
