Looks like a contact mark on the lobe. The ear is not doubled/enlarged. Just a contact mark on that area.

Here is a normal earlobe;
Is-This-Considered-A-Double-Ear?
Same size as your coin. The mark on the lobe is distracting you. If it were a doubled die, that mark would be raised. On yours it is incuse. (damage on the coin)

Yep just a contact mark in the right place to make it look like a doubled ear.
