It's either wear or part of the design, not a doubled ear.

It was the lower part of the earlobe that was making it hard to tell which it was . Thanks for the info!

I think you're just looking at the outline of his ear lobe. Here's a graphic that Coop posted on an old thread. I too was searching the forums for an example of ear doubling on 1988 cents last night. I saved this for future reference.
