I found this today in a roll the R in trust looks doubled, is this a double die? Also the 2 in the date looks hit twice.

Look at this, looks like someone scribbled on it with a nail.. am I right or is this a million dollar Penn that I don't know about?

Looks like PMD (post mint damage) on the R, date, and reverse.

Do the reverse lines stick up above the surface or are they "cut" into it like somebody scratched it with something?

The proper terminology is "Doubled Die".
The die has the doubling on it and each coin minted with that die will show the same doubling.

Yep, the obverse is normal damage from circulation and the reverse is intentional damage from a bored person.
