I've noticed a few of these. At first I thought this was damage from circulation, but it seems a little too clean. What is the deal with this?

It was an issue from the broken '6' on a working hub that created several dies that look like. This is not not PSD. This is from a hub that was breaking down. Some varieties are on some of theses dies created as well a many more that were normal dies without a variety of them.
The hub that creates the die, looks just like the coins created. Hubs have the positive design (Whether master or working hubs). Dies have the negative designs. (Whether Master dies or working dies)

So the issue with the Working hub with the damaged '6', will look just like the coins looked at that time with the same issue. The breaking away of that digit:

Of course die wear will change the copy of the broken '6' issue.

to CCF. It is an error coin, good eye to notice it the way you did. Too bad so many were made.
