2001 Possible Nickel Errors?

Hello, was hoping someone could give me their thoughts on this 2001 nickel. This is my fourth or fifth time buying rolls of nickels and this nickel caught my eye because it honestly looks like a mess.

Above Jefferson's head it appears there is a line going across the top of his head. It also appears the mint mark has a D inside the P and looks like a double die. Also around the inside of the coin seems to be separated from the word Liberty and date. On back of coin the word United the NI are these letters suppose to be connected?

Extra (I think they call it Cud?) by the word Trust. Thank you for your opinions.
