That's from a terribly worn die, I found an '85, just like it, last night.

Actually over all the nickel is not showing a ton of wear, but the die used on the obverse was SHOT. Look at the hair on the back of Jefferson's head. and of course all the extra metal at the edges of the coin. while the reverse of the coin is not showing as much die wear.

Remember there are 2 dies used Anvil (Reverse) & Hammer (Obverse). They are not always replaced at the same time. That is why the the reverse is so much nicer.

Throughtheireyes, damage is a form of wear. Liberty is heavily smashed/worn. Your first pictures show it best.

It's hard to tell in a 2 dimensional photo but actually liberty is not smashed... it's covered with tons of die chips which are raised.. the 7, 8 and mm show the chips are raised more clearly.

I see the chips. It looks to me like the top of liberty has been flattened or worn down. jmo

If you look at picture 3 you can see what's going on... the metal from the edge comes all the way down surrounding the tops of the letters then slopes down from there so yes in a 2 dimensional photo it does look like they have been flattened but in fact the metal from the edge is raised.
