There appears to be a large hole and the coin is bent. Do the grades VF or XF apply in this case?

They do, and whether an actual numeric grade is given seems to depend on the TPG. It looks like ANACS (and ICG ) actually assigns numeric grades to details coins, whereas PCGS and NGC do not.
