Am I just seeing things or is ia D over P nickel and is it worth anything if so?

to CCF. Not possible because the mint stopped punching mint marks by hand in 1990. The mint marks are added to the master die by machines starting in 1990.

and I agree it's not an error but a better picture might help figure out what happened.

Mint marks are not punched on the dies after 1989. So it couldn't be a over mint mark. Since 1990 the mint mark is part of the design of the coin. So this can't happen after 1990. More of a break down of the die issue or coin damage.
