Ok. Let us assume this is not an expressionof love to a lady named XII. The base coin is an AE2 of Theodosius.

Anyone ever seen something like this?

At first I thought a Roman solider might have carved his Legion number on the coin. But Legion XVIII did not exist after the reign of Augustus. Might have been used as some type of gaming piece or a locker tag in a bath house.

I think that Echicento is right, I suppose that they used an obsolete coin to make this token...
My grand father used obsolete copper coins with a mark as count tokens to control the number of olive bags picked up by his farmers...

XVIIII (19), not XVIII (18), but this doesn't change the conclusion.

My first idea was that this was a case of the same Ostrogothic redenomination that somewhat infamously gave us sestertii of 83 nummi.
I think those guys knew their substractive numerals, however (and besides, a denomination of 19 doesn't really fit in their system well).
Even so, if the weight is consistent with that theory, it's a definite possibility.

Other than that... hard to say.
