Yes, it's from the the fourth year of Medmed (Mohammad) V. The date is between the tughra (the monogram) and the inscription on the obverse. In this case, 4 (arba'a). You can see the Arabic numerals in the pic below in case you need that. As for grading, I'm not an expert on that, so I'd go with F-VF.
