I came across this one on ebay, an MS-63 1883 "No Cents" Liberty nickel in a PCGS holder that calls it "With Cents". The seller does properly attribute it.

Makes me feel better about my own quality control.

If that's the worst error PCGS makes I'll have no problems with them.
With probably several thousand coins being slabbed each day an occasional goof can be expected.

Now, grading errors, that's another story.
It's somewhat subjective so opinions will differ.

Look at 1000 PCGS coins and you might find one error, perhaps. You'd probably need to look at more than 1000 to catch NGC.

Although it does happen.

That's still a rate of accuracy I can certainly appreciate.

Actually one in a thousand is not a great rate depending on what you are working with. As I heard it put once, the Saturn V moon rocket had a million moving parts, so at that rate on any given flight one thousand parts would fail.
