1960-D LD LMC "B" Broken On Die?

So I noticed a few things going on with these 1960-D's.
The biggest was the deteriorating "B" in "LIBERTY"; showing typical and really bad.
There are phantom lines on the reverse where there shouldn't be on overlay.
The "M" in "AMERICA" has a die chip or something.

Checked every coin in roll, the lines, the spot on the "M" and the broken "B" are present on every coin in the roll.
The lines and spot near "M" seem consistent but the "B" goes from partial to fully missing.

Just went through 3 rolls of 60Ds, found them with the upper post broke, then lower post broke, then both posts broken, then the lower part of R broken with first lower B, the same with both broke in B. Must kave been a very common problem with the working hub dies.

I hope you are able to establish die progressions with examples in the different varieties. That would make a cool mini-collection.
