PMD (Post Mint Damage). Some type of punch did this and I'm not sure what it is called.

Just PMD, I'm afraid. The fact that the reverse is affected lends credence to this.

Roughly the right ideas so far.Definitely PMD. That penny was partially punched by 2 dies.One was the punch on the obverse and the other was the receiver.No way can a single punch give the sharp crescents on both sides. About the right size for a 3-ring binder paper punch..
