The term "Jock-Strap medals" is used by CIA employees to refer to awards that can only be worn concealed on jock straps. The awards are issued to covert operatives in the CIA. These awards are retained by the CIA until the employee retires. In some cases, the award may never receive it, when security warrants it.
The Bureau of the Mint/US Mint originally manufactured these medals for the CIA. The hallmarks of Gilroy Roberts and/or
Frank Gasparro are on these medal. These hallmarks are left off on later medals manufactured by other manufacturers.
No congressional approval was required to manufacture these medals, as these medals was a procurement (made-to-order) between the CIA and the Bureau of the Mint.
Below is a Career Intelligence Medal with original citation (redacted for privacy of the awardee). The reverse of the medal also has the recipient's name. Note the ribbon material within the case is also used to identify the medal.

The Distinguished Intelligence Medal is to the left.

These CIA medals are "Special Medals for US Government Agencies".
