The B looks doubled but might be PMD. Also an arrow pointing to something in front of him. Any ideas?

The B looks more like MD, or even a shadow(light tricks). Now that in front of the coat is most likely a gouge along the devices edge, not a hub doubled event. Remember the dies are reversed image, the edge of the coat is the face of the die, if something hit or scraped that sharp edge, you have a ding like that. Good eye to pick that out!

I agree with Crazyb0 that it's most likely a Die Gouge. The die probably some how got nicked there.
