Just saw this on a 2006 nevada coin. Within the motto the silver state, the E in silver and the A-T-E seem to not be punched out right. It still has remnants of some die material?

Those are incuse devices. The slightest movement of the dies after the strike will distort them. Note the tops of the letters on the EPU below that area. They are showing Machine Doubling in them. So that is why those incuse devices look weird.
