It seems to be thicker and just barely smaller than regular quarter

Looks like it was stuck in a vending machine, spinning around and around until the repair man came and knocked it out.

It's not an error by any means. This is 100 percent Post Strike Damage (happened after it left the U.S. Mint). You can see the scratched and flattened metal. Worth face value since it's only damage.
