1993-D Jefferson Nickel (Lumpy Texture)

I come across lumpy coins like this one but usually only in the ultra modern era mid 80's till now. Hoping someone can tell me how is it the entire coin can have a crazy lumpy texture, 100% lumpy coverage. Even the outer rim is lumpy. I was thinking late stage die but some of these coins came directly out of Mint sets and would assume the Mint couldn't possibly have put late stage die coins into their sets.

What are the lumps?
What grade would you give it?

I sent in one of these to PCGS as part of a larger order so in around 4 weeks I'll know how they feel about it. Will post the results.

I tilted the nickel up slightly to help give more side view to the lumps.

The surface texture is due to a LDS.

MS-63.
