In the middle on the reverse,that is PSD. The weak reverse is most likely just wear, but could be a normal weak strike for that year.

Looks like a mounted coin, probably glue.

I'm guessing that this cent was used to bypass a fuse in an old fuse panel. The swirl in the middle was caused by the bad fuse being screwed onto it.

Here's an excerpt from a Wiki page:"One form of fuse box abuse was to put a penny in the socket, which defeated overcurrent protection and resulted in a dangerous condition. Such tampering will not be visible without full inspection of the fuse"

It's Post Strike Damage, not an error. It happened after it left the U.S. Mint. The metal has been pushed over and flattened.

Looking at the reverse, it looks like glue sitting
on top of the coin. Probably mounted on something.
