That's severe Die Deterioration Doubling, caused by a severely deteriorated die (die wear, AKA Die Deterioration). http://www.error-ref.com/die-deteri...on-doubling/

That's Die Deterioration Doubling on his nose, caused by a very worn die issue, as mentioned by me above.
