I recently received this token amongst quite a number of other coins given to me by a relative.
It is quite large with a diameter of approximately 60mm and a weight of 62g but doesn't appear to be very old (sorry not sure of the orientation in photos).

I think the orientation is inverted and it is not a real coin. Beyond that...

I did get the following information from another forum which was helpful.

'Obverse:

An emperor's name.

Reverse:

#20998;

The denomination, essentially

Both are texts found on rare cash coins of the Yuan dynasty; so your coin is probably an imitation of one of those, specifically the 2 cash 5 fen coin (see picture No.4):
