It's severe Post Strike Damage ( PSD), caused by Environmental Damage. I would still keep it, since it's a Buffalo nickel.

Could be - just clotted on the surfaces and now starting to peel.

Give it a long soak in acetone - the gunk may come off then you'll have a much nicer example of a buff
