It definitely got the milk spots after being graded. The Canadian mint is famous for milk spots on silver. All the mints have a problem with it now a days, some mints are worse than others though.

Nothing is air tight. PCGS slabs are water resistant which is the closest we have to an air tight slab, but the cost would be astronomical to make an actual air tight vacuum slab.
