If after 1990 there are no more omm and no more rpm is this 1996 d quarter a double die?

Looks to me like the D has taken a hit that flattened it slightly. PMD

Mint mark has taken a well placed hit, causing the metal to get pushed over and flattened. It's Post Strike Damage ( PSD), meaning that it happened after it left the U.S. Mint.
