It is only an error if it was done during the striking of the coin. (Which in this case is not possible) Any damage after that is considered PMD with no extra value.
To CCF! That's just a very beat up and damaged Quarter. It's Post Strike Damage ( PSD), meaning that this happened after it left the U.S. Mint. It's only worth face value.
