It is a die gouge. The fields of the die is the outside face of the die. When a gouge happens on the fields, then the it will disappear behind the die devices, because the devices are deeper into the die. So it is not PSD, this happened during the strike it is a die event. (because it always wasn't there)

No. If is just a die event. Happens often. On a variety coin, it could be a marker. But on a normal coin, it's still a normal coin.

People do collect these and at times like to follow the die through to final deterioration.
