Pretty sure it's only a gaming token and not the real deal, which would be a gold Spade Guinea. The tokens were not made 1790 but later.

George 111 spade guinea gaming tokens are very common.
I have found them in dealers' junk boxes for a dollar.
The hole would detract very little from the value.

I also have a genuine gold spade guinea in about AU55, unfortunately the date (1790) is partly obscured by being ex mounted. That unfortunately reduces it to not much above melt value.
