It's PMD It has taken a hit that has knocked the top part of the 1 over. It would only be an error if it happened during the actual striking of the coin.

The feature at the top may be damage, or it may be a die chip. What I find interesting is what shortened the first 1? It appears to be shorter than it should be. It doesn't extend down as far as the second 1.
