What you call the "square insert" reflects more a strike factor than a grade factor. Poorly struck coins can still be graded BU (or MS).

To me that's a 58 in my collection. If I find it in circulation I don't go over 58. Nice though. Maybe I will learn from these fine fellows I should grade some circulated coins over 58.
