I need some input once again. What would cause the MM to look like this? I would think this has to be PMD.

Question a little vague? But if you talking about the mint mark, it could be vending machine damage, circulation damage, coin counter damage or coin wrapper damage. Incuse marks on on a coin is about 99% of the time just damage to the coin. (they take a beating out there)If that was not was you were asking about, ask another question.

I'm guessing the misshapen appearance of the S was his issue, which could have been caused by many things as you note.

Any one of numerous things could have happened over the last 50 years.
Unless you were their when it got damaged, there is no way to say for sure.
