Very worn and not very round, so I'm just not sure if it is real or PMD. I'd like some opinions before I take the plunge on this one. thanks in advance.

Well it is an 1899, it looks like it was struck through an already struck coin 95% of center.

Possible coinfrog, but then the obverse would show a secondary image as well

Not if there was another planchet in the coining chamber. Then you have a flip over uniface double strike. I would say the offcenter strike came second. However the coin also appears to be damaged with th area opposite the off-center strike having been mashed. It appears to be out of round in that area.

I personally think someone pounded this coin into an already struck cent to create a fake indent. The impact was probably done through a soft cloth or leather and it caused the planchet to expand out of round and the edge details to be obliterated on the obverse. The flattened area on the obverse in particular does not look at all how a genuine indent or double strike would look.

Problem is the lettering in the indent appears to be raised.

You are seeing the indent as the tops of the letters IC, which in that orientation would have to be raised. I was seeing those as the bottoms of the digits 18 in the date, which in that orientation would be incuse. Perhaps the OP can tell which is which and give more detail.

I was going by the lighting, The lettering of ONE CENT is lit from below with the bottom edges of the letter bright and the top edge in shadow. The two characters in the indent also have their top edges in shadow so I assume they are raised as well. If they were incuse the light would be shining into the depression and the top edges would be bright.
