What exactly are the mechanics of it? What has actually happened to cause it?

I've seen 2 recent threads about these. I have one I found last April. I didn't have a clue how it happened or what to call it until I saw the recent threads. It is zinc.

A die cap is when a coin sticks to the hammer(obverse)die, the press continues to strike more coins. Yours is one of the coins struck while the die cap was stuck on the die.

I found one this week, a 1994
