Please Help Identify Error On 2009-P Roosevelt Dime

There's metal missing from the base of his neck and two spots near the rim that I don't believe are worn away. I don't know if it's a Grease Filled Die (unlike any I've seen) or maybe a pre-strike planchet error, or if it's something completely different. I'd appreciate any help. Thank you! Do happen to have an inkling as to the type of flaw? I may be wrong, but it sounded like John1 might of had an idea, hopefully he'll take another look at them without the 2x2. Thanks again.
