It might have been Struck Through a folded Lamination possibly that fell off another coin.

I am gonna go with LAM on this one! Maybe a LAM struck through and then peeled off?
Looks indented to me... it is not raised bump on his head...correct?

Reason I don't think it is as strike-through is how it appeared to ignore striking-through the "GO" in GOD...

A strike through wouldn't change the weight. Would a lamination error of this size cause a difference in weight? I'm not sure, just asking.

Yokozuna, I think it would, since the Struck Through Lamination error (if I'm correct) fell off later.
