If it's raised, I would say die chip..If indented maybe "struck through" or PMD if the reverse is affected (metal movement)

That's your answer, something has hit it creating the "scoop" and pushing up a ridge of metal through the A. It is just Post Strike Damage and has no value over face. It is NOT an error.
