1970- D 25c Washington Quarter: Error/ Variety- Damage/ Doctored?

Good Morning CCF,

Happy Monday to all. Just adding more question over here and hope to gain more knowledge from our coin experts. Have a 1970- D quarter that I was trying to look anything online but still couldnt find one with the same look. I actually have 2 second coin is a 1984 but without the struck through on the head.

* rim is almost flattened down to field surface, looks like been filed all the way around.
* head area that looks like been pushed/ or carved out. But on the Reverse side of that location was raised. Or bump up a little bit
* 5.48g weight
* diameter look normal
* thickness is alittle thinner due to rim
