A couple of things come to mind. But it looks too symmetrical (with "corners") to be a die chip or gouge. As well defined and raised in relief as it is I would expect to see more nearby devices visible if it were a DDO. It appears securely bonded/adhered to the surface of the coin. IMHO, I'm leaning towards a planchet flaw or defect before/during the plating process.
EDIT: Can you provide a clear image of the reverse, please?
