This appears like a Wright and Bale medal but the reverse if different that either B-74 or B-75. Notice the leaves. Anyone have any thoughts as to what this is? it's about 45mm

Might be this one on six bid.

Lot 4009. Washingtoniana Undated (Circa 1834) Wright and Bale Medal. Bronze. 45.1 mm. Baker-75A. Mint State.Obverse with the manufacturer's name partly removed in the die; second reverse.

They are similar but if you look at the leaves on the reverse especially around the words "born" and "aged" they are definatly not the same.

You are correct, yours has the 1st reverse B-74A. I found an example sold on HA.

(1832) George Washington Wright & Bale Medal, First Reverse. Baker-74A. White metal, 45 mm. A left-facing portrait of Washington appears on these medals. An oak wreath appears along the outer edges of the reverse, with text that states, "BORN FEB. 22D. 1732/CHOSEN COMMANDR. IN /CHIEF, JULY 1776/CHOSEN PREST. 1789/ DIED DECR. 14 1799/AGED 68 YEARS.

Baker lists these medals as "V. Rare."

More Information:
Struck in 1832 by the firm of Charles Cushing Wright and James Bale to celebrate the 100th anniversary of George Washington's birth. In late 1833, the partnership between the two would dissolve, leaving relatively few struck. Several years later, the design was resurrected (Baker 75) with the signed obverse having been partially tooled off and the reverse wreath being modified.
